Approved coverages or lines of business for the state of Utah can be found below. Those lines of business marked as being on the “Export List”, do not require evidence of good faith form to be completed. 

If you wish to download a CSV of the export list, hover over the top right corner of the table > click the three dots icon > click Export > CSV > Export
